HR Flexible Work Arrangment Request

Overview

The Human Resources Flexible Work Arrangement Request is used to request remote work and/or shift changes lasting longer than two weeks. These agreements are valid for up to 6 months, and can be renewed at the end of the 6-month period. If you have already submitted an agreement using the Softdocs form, and would like to update or renew that agreement, select the following option under your supervisor information:

Form

Required fields include:

  • Current Office Location
  • Proposed Start Date
  • Proposed End Date
  • Answers to 6 Questions relevant to the request
  • Acknowledgement of Conditions
  • For Flexible Work Schedule Agreements:
    • Complete schedule
  • For Flexible Place
    • Remote work (Yes/No)
    • Which State/Country
    • New Time Zone

If you have a Dual Supervisor

  • Select the Supervisors name from a drop down list, this will populate the following:
    • Dual Supervisor Vandalnumber
    • Dual Supervisor Email

 

Workflow

The Approval Workflow Consists of the following steps:

  • If Job is out of Country

    • Originator Receives email notification to register with the International Programs Office (IPO)

  • Form routed to HR Reviewer

  • Form routed to Supervisor

    • If Dual Supervisor is provided, form routed to dual supervisor

      • Supervisor Reapproves

  • In All cases, If Secondary Supervisor Provided, form routed to Secondary Supervisor

  • HR Reviews Form
    • If Remote work, notify payroll
  • Send approval notifications to Originator and hr-communications@uidaho.edu

Note on Denials: Any supervisor or reviewer can deny a Flexible Work Arrangment Request, it does not require additional review up the approval chain.

Instructions for Supervisor Approvals:

  • If you are the Vice President or Provost, or CEO (or approving/denying on their behalf), indicate below that you are the final reviewer.
  • Otherwise, if you approve the form, provide the name and email of the next reviewer. If you deny the form, the employee will be notified and no further review is needed.
  • If the next reviewer is a Vice President or Provost, please route as follows for the quickest processing:
