Education Records Release Request

Overview

The Education Records Release Request is used by students to provide written consent so that education records can be released on their behalf.

This form is available to All University Students.

Form

  • The form pre-populates the following fields:
    • First and Last Name
    • Vandal Number
    • Email
    • Primary Major
    • Degree
    • College
  • Record Requested
    • Proof of current enrollment (no SSN)
    • Online/non-traditional courses verification
    • Non-Attendance verification
    • Transfer Credit Verification
    • Graduation Application Confirmation
    • Degree confirmation prior to award
    • Degree Confirmation after awarded ($5 fee)
    • Military ID/Insurance Enrollment Verification
  • Expected Graduation Date
  • Attachments if required
  • Purpose or Need for the Record
  • Delivery Method
    • Student Pickup with ID
    • Email
    • Mail

 

Workflow

The Approval Workflow Consists of the following steps:

  • Form is submitted by Student
  • Form routed to Registrar Staff
  • Form routed to Accounts Receivable Staff
  • Student and/or originator are notified via email after action is taken
  • Form exported to Etrieve Content
