Overview
The Request to Waive Late Service Charge form is used by students to request that the Service Charge not be applied to their account after the Registrar Deadline.
This form is available to All University Students.
Form
- The form pre-populates the following fields:
- First and Last Name
- Vandal Number
- Email
- Primary Major
- Degree
- College
- Form submission requires
- Explanation for not meeting Deadline
- Supporting Documents
- Student Signature
Workflow
The Approval Workflow Consists of the following steps:
- Form is submitted by Student
- Form routed to Registrar Graduation Committee
- Form routed to Student Accounts
- Student and/or originator are notified via email after action is taken
- Form exported to Etrieve Content
