Overview
The Human Resources Performance Review - Staff Employee is used by supervisors to evaluate the performance of employees they supervise.
This form is available to All University of Idaho Employees, but will only work to evaluate the performance of direct reports. If an employee has dual supervisors, please have the primary supervisor complete the form and include the second supervisor in the Dual Reporting Supervisor section.
If you need to evaluate the performance of an employee that is not a direct report, please submit a request for access to the Proxy HR Staff Performance Review form from the EA Application Administration team here.
Form
The form automatically populates the supervisors Name, Vandalnumber and Email.
This section is followed by an Employee Lookup drop down, which will contain employees under that supervisor. Upon selecting an employee, information about that employee, as well as the next supervisor will be automatically populated.
Required fields include:
- Evaluation Type
- Annual
- 3-Month Probation
- 6-Month Probation Reporting
- Period Start Date
- Period End Date
- Performance Level Rating
- Needs Improvement
- Meets/Exceeds Requirements
- Definitions of these ratings can be found here
- Compliance Training Completed (Yes/No)
- Safety (Yes/No)
- General Comments
- Comments Supporting "Needs Improvement" if applicable
- Future Expectation/Goals
