The Registrar Consent for Release of Student Information is used to give consent to individuals to obtain authorized information, or remove all existing consent.
This form is available to All University Students.

- Financial Aid Information
- University Housing Information
- Purpose of authorization
- Authorized Party Information
- Checkbox with authorization statement
Workflow
The Approval Workflow Consists of the following steps:
- Form is submitted and Registrar Office Admin
- Form approved or denied
- No notifications are enabled on approval or denial.
Document Storage
The final document is stored in Etrieve Content.
