Replacing a Lost Vandal Card

This article explains how to request a replacement for a lost Vandal card using MyUI and the Vandal Card form.

Note, deactivating your lost card is optional as once a new one is made the old one will become automatically deactivated. However, if your card contains permissions that are a security risk (I.e., access to authorization needed buildings) it is recommended that you deactivate your card as soon as it is lost.

You can deactivate your Vandal Card via the following steps:

  1. Log into MyUI
  2. Find the card labeled "Vandal Card" either in the "Resources" tab or in "View All Cards" search.

  1. In the Vandal Card card select "Deactivate Card" link.

  1. In the new window select the Vandal Card listed and select the "Deactivate" button.

Requesting Replacement Card

When replacing a lost card you will need to follow the provided steps as it will not be the same as when you requested your first Vandal Card. If you attempt to request your lost Vandal Card as a broken card it will not be printed without the physical card being shown as dysfunctional. 

 

Pay the Lost Card Fee

Before requesting a lost card replacement you will need to pay the lost card fee as lost cards will not be printed until a receipt is received. Note, that when paying the fee it is preferred that you include your Vandal email as the email identification so your receipt can be identifiable before your card is printed.

Request Replacement Card 

Once you have paid the lost card fee you can begin the process of requesting a new card by filling out the following request form. When filling it out set the request reason as a lost card replacement and if wanted you can update the current photo for your Vandal Card (Photo requirements can be found here). If you would like to receive the card at a personal address instead of the Student Technology Center's office in TLC 128, specify it in the form (I.e., 123 Happy Street, Boise ID 83709).
