This form is used by students to report a name change.

This form is available to All University Students.

Form

  • The form pre-populates the following fields:
    • Former Name
    • Vandal Number
    • College
    • College Email
  • Form requires the following information
    • Semester
    • New Legal Last Name
    • New Legal Middle Name
    • New Legal First Name
  • Documentation is required to be attached to the form
    • Certified Court Order granting name change
    • Marriage Certificate
    • Passport
    • Driver's License
    • Permanent Resident Card
  • Form submission requires
    • Acknowledgement of Guidelines

Workflow

The Approval Workflow Consists of the following steps:

  • Form is submitted by Student
  • Form routed to Registrar Staff
  • Student and/or originator are notified via email after action is taken
  • Notifications sent to the following recipients
    • Financial Aid
    • Graduate Admissions
    • Payroll
    • Human Resources
    • Independent Study in Idaho
    • Admissions, Application
  • Form exported to Etrieve Content
