- The form pre-populates the following fields:
- Former Name
- Vandal Number
- College
- College Email
- Form requires the following information
- Semester
- New Legal Last Name
- New Legal Middle Name
- New Legal First Name
- Documentation is required to be attached to the form
- Certified Court Order granting name change
- Marriage Certificate
- Passport
- Driver's License
- Permanent Resident Card
- Form submission requires
- Acknowledgement of Guidelines
Workflow
The Approval Workflow Consists of the following steps:
- Form is submitted by Student
- Form routed to Registrar Staff
- Student and/or originator are notified via email after action is taken
- Notifications sent to the following recipients
- Financial Aid
- Graduate Admissions
- Payroll
- Human Resources
- Independent Study in Idaho
- Admissions, Application
- Form exported to Etrieve Content
