Overview

The Provost Special Course and Lab Fee Request is used to request a brand new course fee, change an existing fee, or discontinue an existing fee.

This form is available to All University of Idaho Employees.

Form

Special course fees may only be charged to cover the direct costs of the additional and necessary expenses that are unique to the course. This includes the costs for lab materials and supplies, specialized software, cost for distance and/or online delivery, and personnel costs for a lab manager. A special course fee shall not subsidize other courses, programs or institution operations.

  • Required fields include:
    • Submitter Information
      • Department
      • College
    • Department Contact
      • V# of College Contact
      • First Name, Last Name and Dept Contact Email populate after V# is selected
    • Additional Information
      • Course Title
      • Course Subject
      • Course Number (Include Cross Listed)
      • Effective Semester
      • A checkbox for the proposed use for the requested fee
      • Rationale for fee
      • Reason for request
  • The form checks that valid emails are provided for approvers, and present as users in the Etrieve system.
    • Department Chair
    • Fiscal Officer
    • College/Associate Dean

 

Workflow

The Approval Workflow Consists of the following steps:

  • Form is submitted and routed to Department Chair for Approval
  • Form routed to Fiscal Officer
  • Form routed to College Dean
  • Form routed to SCLF Committee
    • If form approved
      • Notification sent to Originator
      • Notification sent to Department Contact
      • Notification sent to Accounts Receivable
    • If form denied
      • Notification sent to Department Contact

 

Document Storage

The final document is not stored in Etrieve Content.
